There are many factors that determine your eligibility including your location, age, and whether you meet the requirements for disability or early intervention support.

• Be an Australian citizen, permanent resident or hold a protected special category visa.
• Must be under 65 years of age.
• You must be in the area where NDIS is available.

• Help you with pre-assessment.
• Reports requires stating personal details such as: name, age, primary disability and dwelling place.
• Personal background in light of current support system.
• Current circumstances to deal with daily life.
• 12-month Goal setting (work, social, education and community)
• Areas that you would like to focus on.

After the plan is devised we will move on the next phase of management. You can choose from one of the following options for managing:

• You or your nominee (parents)
• A registered plan management provider
• The NDIA

If you opt for a plan management provider, favour and care will be there to help you with financial intermediary. Our Finance team can aid with the monitoring of your fund, pay invoices and handle all the other financial elements of your package.

Our NDIS Support Coordination team can help coordinate your supports. We’ll collaborate with you to determine what type of supports you’ll need in your package, providers for those particular supports, costs involved, location of the providers and aid with the development of service agreements.

Now is the time to start the service delivery as all the elements have been established: supports you require, workers/care takers who will be delivering them, time, location and costs involved.

Favour and care will be there at each and every step to manoeuvre through the journey as smooth as possible. We will customize the plans according to your needs to make sure you are achieving the goals you have set for yourself. All the activities will be monitored, and reports will be generated and shared to determine the success factors.

We will take the worry off you as all the issues will be handled by our plan management team particularly the financial matters. Favour and care will make sure you are on top of things by generating timely reports.Plan Review

As the plan will be for 12 -36 Months, we will review to see if circumstances have changed. It will be the time when you have to consider all the aspects on what goals you have achieved and what can be done in the next 12 months to make it much better for you. We will also prepare you for your meeting with NDIA when time comes for your review.
